NYS Health Examination Form
Required for grades: K, 1st, 3rd, 5th and 7th. If you child is not in this grade, please be sure to fill out the ParentSquare Form "Student Health History Form" to ensure we have updated information.

Permission to Administer Medication
Pursuant to the New York State Education Department's Regulations, any and all medications dispersed while student is on school grounds must be "prescribed" / authorized by the student's physician.
